Scheduling
An Appointment
Advanced Surgeons,
P.C. sees patients Monday through Friday, 8:30 a.m. until 4:30
p.m. To schedule an appointment call our office during
these hours. When your condition requires urgent attention, we
will make every effort to meet your needs. In return, please be
on time for your appointment. If you need to cancel an appointment,
please call our office at the number below as far in advance as
possible to allow for the substitution of other patients.
When
You Arrive At Our Offices
On your first visit, we ask that you complete
a brief medical history and registration form. Also, please bring
any pertinent information with you including medical records, x-rays,
and insurance cards. If your insurance requires a referral from
your primary-care physician, please be sure that this is arranged
prior to your arrival. Click here to obtain
copies of the patient forms that you may fill out prior to your
appointment. All medical records and any information given to our
staff will remain confidential. No information will be released
unless written authorization from you is received. To see our privacy

Telephone
Consultations
If you need to speak with your doctor, please make
non-emergency calls during regular office hours when your medical
records are available. Our receptionist will take some preliminary
information and let you know when to expect a return call. If you
are requesting that a prescription be refilled, please be sure to
call during office hours with the pharmacy's phone number on hand.
Our answering service will respond to calls when the office is closed.
Non-emergency messages may also be left with this service. All calls
will be returned as soon as possible.
After Hours
Assistance
All doctors alternate weekend and evening
call duty in order to provide medical care after business hours.
Our answering service will forward emergency messages concerning
your needs to the physician on call.
Insurance
Claims
Our office will be happy to file your
insurance claims. However, you will be responsible for any deductible
amounts that your policy may require. All co-pay or co-insurance
amounts are due at the time of service; any prior arrangements
must be made with our patient accounts representative.
Please understand that your insurance is a contract between you
and your insurance provider. Therefore, it is your responsibility
to understand your coverage and exceptions to your policy. Because
of this relationship, you are ultimately responsible for your
bill.
